For people with a life-limiting illness of six months or less and who desire no further curative measures, such as chemo or radiation therapy, Touchstone Health’s hospice care can help you and your family or loved ones through the referral process. We often describe ourselves as a “hospital without walls” because we employ a broad network of healthcare services from the beginning to the end of the journey. Our intimate knowledge of where you’ve been and where you want to go helps us provide the care you need right now. Simply, we know your journey and can walk it with you.

Whether in your home, a nursing facility or a hospital, depending on the circumstances of each individual patient, our hospice team includes your personal physician, the hospice medical director, nurse, home health aides, social workers, chaplain/counselors and compassionate, trained volunteers who work together to provide the specific needs of each individual patient.

• Medications related to the patient’s comfort needs or terminal diagnosis
